Once you submit the form, our Care Coordinator will contact you by phone within 1 business day for your free discovery call where you will learn more about our clinic and answer any questions you may have. (Be sure to check your SPAM folder for emails.) We use an online portal to track all appointments, records and communication. If you decide that you would like to move forward after your call, you will receive an email containing the link to set up your account in the Patient Portal, and be one step closer to getting the expert guidance you deserve!
